Changes to proof of COVID-19 vaccination requirements

From Monday, 31 January proof of vaccination requirements will be required upon entry to our hospital.

Persons seeking to enter Western Australia hospitals from Monday, 31 January 2022 can show proof of vaccination either in a digital or paper based form, along with acceptable forms of identification if they do not have the ServiceWA app to show proof.

Check In apps

Visitors must scan and register with the hospital’s QR code as well as the ServiceWA app QR code displayed at the hospital’s entrance/s before entry; or provide their contact details to the hospital representative for registration assistance.

Proof of vaccination

Visitors aged 16 years and over must be fully vaccinated for COVID-19 (with two doses of a TGA approved COVID-19 vaccine), and need to show one of the following before entry as evidence of vaccination as identified by the WA Government:

  • COVID-19 digital vaccination certificate via the ServiceWA app, Medicare app or My Health Record; or
  • Printed copy of COVID-19 vaccination certificate; or
  • International COVID-19 vaccination certificate

16 years and over
Visitors aged 16 years and over who have not had the prescribed, mandatory COVID-19 vaccines are not permitted to enter the hospital. Some exceptions for medical treatment may apply and must be approved by the hospital prior to visiting.

Under 16 years
Visiting restrictions do not apply to children under 16 years of age. In accordance with hospital policy, children under 16 years of age must be accompanied by a responsible adult at all times. In accordance with WA Government requirements this accompanying adult must be fully vaccinated.

With a medical exemption
Evidence of a valid medical exemption must be provided if requested by the hospital. Acceptable forms as identified the WA Government are a medical exemption recorded on the Australian Immunisation Register and displayed in the person’s Immunisation History Statement along with a medical certificate issued by their medical practitioner.

Visitors with a medical exemption will be requested to wear a single use surgical face mask, covering the nose and mouth at all times.

Unvaccinated visitors who present with a negative PCR test result will not be permitted entry to the hospital as this does not meet WA Government requirements.

Unvaccinated visitors

Failure to provide evidence of COVID-19 vaccination status or medical exemption

In accordance with WA Government directives, visitors who refuse or are unable to provide an acceptable form of evidence will not be permitted entry to the hospital. Visitors who refuse to leave the premises or forcibly enter the premises may be referred to WA Police Services for intervention.

In addition, visitors not permitted to enter the hospital is anyone who:

  • is unwell
  • has been diagnosed with COVID-19 or asked to quarantine
  • is in home quarantine, unless granted an exemption
  • has returned from overseas in the last 14 days (excluding safe travel zone countries), unless granted an exemption
  • has had contact with a person with COVID-19 in the last 14 days
  • has visited a COVID-19 hotspot in the last 14 days or since the hotspot was declared (whichever is shorter)
  • has visited an interstate exposure venue in the last 14 days unless an exemption has been granted for an end of life visit
  • has been in an interstate area of concern in the last 14 days or since the identified start date (whichever is shorter), unless they have received a negative COVID-19 test since entering Western Australia.
  • has been tested for COVID-19 and are waiting to receive the results (except for tests due to surveillance testing obligations)
  • has COVID-19 symptoms of fever (37.5 degrees or more), cough, shortness of breath, sore throat, loss of smell or taste, runny nose, diarrhoea, nausea, vomiting or fatigue.
